Annual fee

Covers standard forms, letters and notes, copies of records, and cosmetic cryotherapy for a set number of lesions per year.
Every item is also available individually. The fee amount and a written list of what it covers are provided before payment.

Fees are provided in writing before any service begins.
Payment is taken at each visit; instalments can be arranged.
Receipts are provided. Whether a service is reimbursable depends on your individual plan; please check with your provider.
Insured medical services are billed to the Medical Services Plan and not to the patient. The fees above are for services the Medical Services Plan does not cover. Choosing not to pay for an uninsured treatment does not affect the insured medical care you receive. Results vary from person to person and no outcome is guaranteed.
