The enquiry form collects your name, your email address, your telephone number, the general subject of your enquiry, and the time of day you prefer to be contacted. Nothing else is collected through this website.

No health information is collected through this website. The form does not ask for symptoms, medical history, diagnoses or medications, and you are asked not to include them.

Enquiries are delivered to a clinic email account and are read only by the clinic. They are not sent to a marketing platform.

The clinic email service is provided by Microsoft, with data held in Canada. Your medical record, where one exists, is held separately in the clinic's electronic medical record and is not connected to this website.

Enquiries that do not lead to an appointment are deleted within twelve months. Where an enquiry leads to care, the relevant information is transferred to your clinical record and retained under the retention requirements of the College of Physicians and Surgeons of British Columbia.

Your information is used only to respond to your enquiry and to arrange an appointment. It is never sold, rented, or used for marketing, and it is not shared with third parties except where required by law.

This website uses no analytics that record the contents of form fields, no session recording, no heatmaps and no chat widgets.

Under British Columbia's Personal Information Protection Act you may request access to the personal information the clinic holds about you, ask for it to be corrected, or ask for information collected through this website to be deleted. Requests are answered within thirty business days.
